How do I make an Appointment?

Simply call our main number at between 8:00 a.m. and 5:00 p.m., Monday through Friday. Or you can request an appointment online using this form.

How long will it take me to get an appointment?

For urgent conditions we will see you within 1-2 days. For conditions that are non-urgent, you can usually be seen within one week.

What should I bring to my appointment?

Bring your driver licensee or photo ID. Also bring any cd, x-ray film and reports. Many patients find that bringing comfortable clothes, such as gym shorts, is preferable to wearing paper gowns when being seen for back, hip, or knee problems.

How much will my office visit cost?

We are not contracted with any medical insurance planes including Medicare, therefore the cost of your visit will be paid in full at the time of visit. We will provide the needed information so you can file with your own insurance, we will not be filing with your insurance. Please call office for details 281 888 3416.

How do you handle prescriptions?

For your safety, prescription refills are authorized during regular office hours only, when we have access to your file and records. We ask that you call your pharmacist 24-48 hours in advance for a refill, and he or she will then contact us for authorization. You can also request a prescription refill online using this form. Please note that only active prescriptions approved by your doctor will honored when submitting your request online.

How do you handle telephone calls?

Our highly trained registered nurses and physician assistants can usually answer most of your questions when you call. Since the doctors and medical assistants are often in surgery or seeing patients, you may be asked to leave a message, and can expect a return call by the end of the day. For emergencies, a doctor will be paged and return your call as soon as possible.
